About Hazel

Hazel Zhakata is the Founder, Registered Manager, and Nominated Individual of Oakevale Healthcare Ltd. With more than 22 years of continuous experience in nursing, health, and social care, Hazel has built a career dedicated to helping people live with dignity, independence, and choice.

Hazel qualified as a Registered Nurse in 2003 and has maintained active registration with the Nursing and Midwifery Council (NMC) ever since. Over the years, she has worked across hospitals, community services, and agency nursing, gaining extensive experience in medical, surgical, elderly, and complex care. Even while managing services, Hazel continued practicing as a nurse, ensuring her clinical expertise remained strong and up to date.

From 2019 to 2024, Hazel managed supported living and domiciliary care services at Adriel Care Ltd, where she was also a CQC Registered Manager (2021–2024). During that time, she received highly positive feedback from service users and families, with commendations shared directly with CQC. In 2024, Hazel voluntarily stepped away from that registration to establish her own company, Oakevale Healthcare Ltd, bringing together her nursing background, leadership expertise, and vision for a high-quality domiciliary care service.

Hazel also holds a Level 5 Diploma in Leadership for Health & Social Care and Business Management and completed Top Flight Secretarial Studies (1996–1999), giving her a strong foundation in administration, governance, and compliance. She has also undertaken an extensive portfolio of training, including:

Today, Hazel leads Oakevale Healthcare Ltd with a clear mission: to deliver safe, effective, caring, responsive, and well-led domiciliary care for children, young people, and adults aged 0–65 across Dorset and surrounding areas.

Her leadership combines clinical expertise, proven management skills, and a strong commitment to person-centred care. Hazel’s vision for Oakevale is to provide a service that families and commissioners can trust, and one where staff feel valued, supported, and inspired to deliver the highest standards of care.
